MEDICAL PROVIDERS: When billing for a problem oriented office visit and a Routine Physical on the same day - BE SURE YOUR DOCUMENTATION supports each encounter. Patients in the VERY NEAR future will have access to their medical records and they will question. If you're not able to give a supporting response they will seek a response from their insurance company. The insurance company may request medical records and possibly schedule an unplanned audit. So cover yourself – DOCUMENT your encounters correctly and completely to keep your reimbursement. In additon, billing for 2 office visits on the same day can generate 2 office visit copays. This is confusing to patients. While treating the patient for the problem oriented office visit on the same day as a routine physical is a convenience for the patient; it requires knowledgeable staff members to review the claim and ensure it goes out with the correct modifiers and code linkage so both claims will pay. Once the insurance carrier has paid both visits, the payment must be posted correctly to patient account. If the Insurance carrier did not pay 100% of the allowable charge after the copay on the problem oriented visit, the remainder of that allowed amount should be written off. However, according to the insurance carriers guidelines the patient IS responsible for the 2nd copay. Many patients will not want to pay this 2nd copay because most don't even realize they are receiving 2 services on the same day. After explaination some will be willing to pay the additional copay and some will not. Some won't ask and will just pay their bill. My suggestion is write-off the second copay; after all you've been paid by the insurance carrier for the second visit therfore you're gaining revenue and losing very little. If the statement goes out with the second copay charge and the patient inquires about the charge- PLEASE EXPLAIN the charge to them. Patient education gpes a long way and they will be more willing to pay if they understand what the charge is for. Patients are SMART and want to wanted, appreciated and informed.
