Preventing Insanitary Conditions at Compounding Pharmacies

Compounding pharmacies prepare compounded medications for patients who have medical needs that cannot be met by standard prescription drugs.  However, compounding pharmacies have a bad reputation in the United States due to a deadly event that occurred seven years ago. In 2012, a compounding pharmacyexposed 13,000 patients to fungal meningitis. New England Compounding Center, which prepared steroid shots for patients nationwide, recalled more than 2,400 compounded drugs in 50 states after the deadly outbreak. Seventy-six patients died due to the tainted syringes.   The outbreak prompted federal regulators at the Food & Drug Administration (FDA) to publish advisory notices that more clearly define the types of insanitary conditions that could harm patients of compounded pharmacies.  Federal lawmakers amended the Food, Drug and Cosmetic Act to reduce the chances of future occurrences of drug adulteration at compounding pharmacies. Under Section 503A of the law, some compounding pharmacies are exempt from following certain manufacturing practices.  However, there are no exemptions for these pharmacies from 501(a)(2)(A) of the act. This part of the act defines what constitutes an adulterated compounded drug. If compounded medications are prepared in insanitary conditions, then the law considers them to be adulterated.  What Are Examples of Insanitary Conditions for Sterile and Non-Sterile Drugs?  There are multiple examples of what constitutes “insanitary conditions” at sterile and non-sterile production areas at a compounding pharmacy. Examples listed in the FDA’s guidance document include:  ·      Vermin, such as flies, cockroaches or other insects ·      Dogs, cats and other animals  ·      Evidence of microbial contamination, which may be visible ·      Evidence of non-microbial contamination, such as paint chips, rust, hair and other debris ·      Preparing compounded medication with active construction nearby ·      Standing water or leaks from water sources, such as pipes ·      Failing to properly clean drug production areas and utensils ·      Improper cross-contamination prevention methods ·      Using active and inactive ingredients with more impurities than similar commercial-grade ingredients Even though non-sterile drugs are not required to be sterile, the presence of microorganisms or other impurities can still lead to significant harm.  What Are Examples of Insanitary Conditions for Sterile Drugs?  Sterile drugs have stricter requirements, and for good reasons. Impurities in sterile compounded drugs are much more likely to harm patients. The FDA lists examples of insanitary conditions that are specific to sterile compounding. Examples of insanitary conditions for sterile compounding include but are not limited to:  ·      Applying a gown in a non-classified area ·      Entering and exiting non-classified areas without replacing equipment (such as gowns) ·      Incorrectly disinfecting supplies as they are moved from high-risk to low-risk environments ·      Failing to properly disinfect gloved hands, such as after touching potentially contaminated objects ·      Moving too quickly in an ISO Class 5 clean room What Are Examples of Insanitary Compounding Procedures?  The FDA’s guidance document also lists specific procedures that are insanitary. Procedures the FDA lists include:  ·      Failing to conduct regular environmental monitoring ·      Failing to conduct personnel sampling ·      Failing to perform necessary filter-integrity testing methods ·      Using sporicidal agents frequently and incorrectly ·      Use of non-sterile disinfecting agents in a classified area and incorrectly using sterile disinfectants The FDA regularly inspects 503A compounding facilities. During these inspections, regulators are looking for insanitary conditions that could put patients at risk of suffering harm.  However, operators of compounding facilities must realize that the FDA’s guidance document is not an exhaustive list of what may constitute an insanitary condition.
