My name is Catherine Hariett Meiking. I was born on 11/14/1944 and removed from my mother's custody in March of 1950. I have little or no memory of any of my childhood. I am giving you all of the information that I have from my birth certificate and attached "Affidavit and Certificate of Correction", in hopes that you can help me trace my childhood and life until prestent. I have no recollection, even up until the age of 18. On original Birth Certificate my father's name is "Legally Omitted". There is an additional note on Birth Cert. between line #22 & #23 - "What treatment of child's eye at birth? - 1 of Silver Nitrate." *I am blind in my left eye. A blood test for Syphillis administered to "the mother of the child" on 08-25-1944 by "Labrotory" B.C. Penile, M.D. The results of test were omitted on the Certificate, signed by Kate Lofito, 11/21/1944, 1410 W. Harrison St., Chicago; phone Hay 7761, filed 12/205/1944. Please help me find my life's records and information by mailing me any records you may have and/or direct me to how I can obtain them and complete my adolecent history, moving forward.
