Midlife Weight Loss Cuts Heart RiskBy ANAHAD O'CONNORBeing overweight as a teenager carries a greater risk of being an overweight adult, along with an increased risk of dying of heart disease. But overweight adolescents who slim down in middle age may lower their risk, a new study shows.Previous studies have linked being overweight as a teenager or young adult to deadly consequences later in life, including an increased risk of heart disease. But it was unclear if the risk was higher simply because heavy teenagers become heavy adults, or because being overweight or obese as a young adult causes irreversible damage.To find out, a team of scientists at Harvard Medical School and elsewhere combed through data from the Harvard Alumni Health Study, which tracked the medical information of men who entered the university as freshmen from 1916 to 1950. The researchers gathered data on about 19,000 of the Harvard alumni who were subsequently followed in later decades, looking at their habits, heart disease risk histories and body mass indexes, among other things. The subjects were followed for up to 82 years.The study, published on Monday in The Archives of Internal Medicine, found that the heaviest students were more likely to become overweight or obese adults. Furthermore, the men who were obese as freshmen had nearly twice the risk of dying of heart disease decades later as the men who had been of normal weight in college. A similarly increased risk was seen among the men who were overweight as freshmen.“That simply reflects the fact that fat kids become fat adults,” said Dr. I-Min Lee, an associate professor of medicine at Harvard Medical School and an author of the study, “and that drives deaths from heart disease.”But when the researchers factored in middle age, adjusting for the subjects’ weight at that time, they found that the risk changed. The men who started college overweight or obese but were of normal weight in middle age no longer had a higher risk of dying of heart disease.Body mass index in middle age was a strong predictor of dying of heart disease, the study showed. Being overweight in middle age increased the risk 25 percent, and being obese raised it 60 percent.While the study looked only at men, Dr. Lee said she believed the findings would hold for women as well, since the biological effects of overweight and obesity on heart disease risk are similar in the two sexes. In both men and women, the data linking obesity in middle age to heart disease is clear, she said.The only other study that prospectively measured B.M.I. in both early and later life, published over 20 years ago, showed that a higher body weight in adolescence raised the risk of heart disease later in life, regardless of B.M.I. in midlife. But that study was considerably smaller and looked at only about 500 people.Dr. Lee said that while the latest study offered good news, “it’s not that it’s not harmful to be fat when you’re younger,” since extra weight in adolescence tends to follow you into adulthood. Confronting Childhood Obesity

How parents can confront childhood obesity by fostering environmental change in their communities Excess weight and obesity in childhood and adolescence has become a serious public health concern in the United States. The “obesity epidemic” has particularly exploded in the last 25 years. According to the Centers for Disease Control and Prevention, the prevalence of obesity has more than doubled among children ages 2 to 5 (5.0% to 12.4%) and ages 6 to 11 (6.5% to 17.0%). In teens ages 12 to 19, prevalence rates have tripled (5.0% to 17.6%).This has significant implications for the life expectancy of obese youth who are likely to experience higher rates of hypertension, elevated cholesterol, and Type 2 diabetes than their parents. This may be the first generation to have a shorter life expectancy than the one immediately before them. There is some encouraging news that growth in childhood obesity prevalence has slowed in recent years, however serious efforts are still required to fully reverse the trend.So what can be done? It is well established that changes to diet and physical activity are key to combating obesity. However, changes to environmental conditions are rarely discussed. Obesity reflects the complex mix of influences that both genetics and environment have on physical health. It is no accident that there are disproportionately higher rates of obesity among children and families living in underserved communities where environmental resources to support healthy behaviors are scarce.Environments (particularly in underserved communities) can undermine healthy behaviors in the following ways:There is a lack of ready access to healthy foods (i.e., fruits, vegetables, whole grains, and lean protein). Families in poorer communities often live in “food deserts” where supermarkets and grocery stores are scarce or charge higher prices for healthy foods than processed foods. They must often rely on convenience stores and small neighborhood stores that offer few, if any, healthy food choices. To make matters worse, many underserved communities, particularly in urban areas, have an overabundance of fast food establishments that are often located near schools and playgrounds.The built environment in poorer communities (with fewer parks, bike lanes, playgrounds, recreational facilities, or walkways) restricts opportunities for children and teens to get exercise.Families living in high crime neighborhoods are understandably reluctant to allow children and teens outside of the home to play or exercise.Underresourced schools in many communities have eliminated or cut back on physical education, an invaluable way to introduce exercise into children’s lives.What can parents do to push for change in their local environments?Work with local or city governmentLet’s Move - the federal government’s new initiative against childhood obesity - is devoting $400 million in grants to bring grocery stores and farmers’ markets to underserved communities. Parents can encourage their local leaders to apply for these grants. Parents can also push for the introduction of affordable transportation (e.g., bus or shuttle lines) to supermarkets or grocery stores currently located outside their communities. Parents can call for local leaders to improve their land use policies by encouraging the construction of parks or playgrounds and restricting further encroachment by fast food establishments into their neighborhoods. Local governments can also promote the use of vacant land for community gardens or farmers’ markets. Parents can advocate for increased community policing to enhance neighborhood safety for their children.Work with schools and the local communitySchools are ideal sites for community gardens from which healthy fruits and vegetables can be harvested and sold cheaply to local residents. These gardens can be used to benefit children’s education as they learn about basic biology as well as healthy eating and nutrition.Parents can push for more after-school programs that incorporate physical activity or nutrition education.Parents can pressure schools to eliminate the use of vending machines on school grounds.Parents along with members of the community can volunteer to coach afterschool sports (e.g., Little League, softball, basketball, etc).In the home - the most immediate environment in which to impact the child’s dietary habits and preferences.What parents can do:Incorporate healthier foods into the family’s diet (e.g., fruits, vegetables, whole grains, and lean protein) and limit consumption of processed foods (e.g., sugary beverages and meals with high amounts of refined sugar, starch, salt, or fat).Sit down to enjoy nutritious meals with their children. Sciatica. Disc herniation or peripheral nerve entrapment?

Sciatica is a common form of low back pain. Classically, sciatica means the patient has pain along the distribution of the sciatic nerve--down your buttocks, through your thigh, down the back of your leg, or right down to the foot and heel. Almost everyone in the medical community believes that sciatica comes from a disc herniation in your lower back. I agree that that is one of the locations that the sciatic nerve can be entrapped, but there are also many other locations the sciatic nerve can be entrapped. It can be entrapped between the muscles of the hamstrings, between the thigh adductors and hamstring muscles, at the superior gemellus muscle where the sciatic nerve passes over the muscle, and at the piriformis muscle where the sciatic nerve passes under or through the muscle. If the practitioner is treating a lumbar disc for sciatic and the problem isn't at that location, the patient won't get any better. I find the exact location the sciatic nerve is being irritated and I break up the adhesions and scar tissue that has built up around that location. You can't just look at the disc, you have to look along the entire path of the sciatic nerve in order to find the problem and then fix it. Active Release Technique (ART) is the perfect solution for these types of problems. I'm one of 30 in the state of Minnesota certified in ART. You don't have to live with the pain anymore. If you've been everywhere else and haven't tried ART, you haven't tried everything. My number one priority in my clinic is to get people better, and one of the ways I can do that is to get the word out about Active Release Technique and how it can help you.
