My insurance company requires a pre-authorization from the physician in order to cover certain medications and injections. On my second office visit, the injection was ordered for me by office personel as well as the pre authorization was taken care of, and they even had the injection delivered to the doctor's office. Three months later when the injection was due again, I was told that they never ordered anything for me and that my insurance has been nothing but a problem since the beginning.
