How to Change Your Name on a Naturalization Certificate

Your naturalization certificate is a legal document. If you ever need to change the name on your record, you can do so by filling out the appropriate paperwork and submitting a payment. Changing your naturalization certificate will take some time to process, but is an important step.

Items Needed:

• Form N-565


• Black ink pen


• Documentation to support the name change


• Payment

Instructions:

1. Obtain Form N-565 from the United States Citizenship and Immigration office. You can request it by mail or you can download and print it from the office's website.


2. Fill out the form using a black ink pen. Complete all of the questions to ensure that your form can be processed as quickly as possible.


3. Make photocopies of documentation to support the requested name change. This may be a copy of your marriage license, for example, or other court documents that show a legal name change.


4. Mail your completed form, check or money order for payment and documentation paperwork to the address indicated on the form. Depending on your state of residence, you may be sending the form to either the Nebraska office or the Texas office. The current charge to process the form is listed on the form. Your name change cannot be completed if you do not include payment with your form.


5. Wait for the corrected naturalization form to arrive in the mail. If the Citizenship and Immigration Services office has additional questions or needs additional information, they will contact you to obtain the information.
