Medication Administration Monitor blood pressure, pulse, respiration, and blood glucose Perform transfers (use mechanical lifts, if necessary) Drive to appointments, run errands, organize social outings as needed Light housekeeping duties and meal preparation Simple dressing changes Administer oxygen, do catheter care, and tube feedings Ensure safety for clients with memory loss Provide companionship and help prevent falls Our Registered Nurses and Licensed Practical Nurses Wound Care Medication management: Set-up of medications Skilled observation and assessment Train, supervise, and consult with Home Health Aides for quality of care In-home Client visits, which include client education Develop personalized plans of care Provide case management and advocate for clients to ensure the highest quality of care at home Act as a liaison between (families, guardians, doctors, and community workers) Arrange for care and coordinate with other providers (e.g. Medical Equipment Suppliers, Pharmacies, Medicare or Hospice) Are on-call 24 hours a day Live-In-Services These services include both medical and non-medical care provided by our well trained and certified Home Health Aides.
